What is Autosomal recessive PKD?


Polycystic kidney disease or kidney cyst is a genetic disorder resulting in the development of thousands of cysts over the kidneys. PKD enlarges the kidney function and deteriorates renal capacity. PKD also causes cysts to develop somewhere in your liver and anywhere else in the body. In addition, PKD leads to high blood pressure and kidney failure when left untreated.

There are two types of PKD depending upon the inheritance pattern and genetic mutations; Autosomal dominant PKD and Autosomal recessive PKD. In this blog, we are going to talk about ARPKD more. 


What is ARPKD?

The autosomal dominant form of PKD is seen more in children. During ARPKD, the liver is also affected. The affected gene is PKHD1 on chromosome 6. Children with ARPKD may have a defect in the protein that allows producing PKHD 1 gene. The required protein is required for the structural development of the kidney. Can ARPKD be diagnosed during pregnancy?


The sonography test done during pregnancy may show very decreased fluid. This indicates that the kidneys have not been adequately functioning. The other subsiding organs of such babies often do not develop properly leading to breathing issues. 


What happens after delivery?

The babies born with ARPKD may have breathing issues due to underdeveloped lungs and large kidneys. This could be life-threatening. Kidney failure is also common in babies born with kidney cysts. 


If the baby survived the early days of life, there are high chances that he will survive until adulthood though severe complications would be there. At times, children get diagnosed with kidney cysts later in life when they undergo tests for blood in the urine or high blood pressure. Around one-half of the children have kidneys failed by 10 years and the rest of the children by 20 years of age. 


What are the early signs of ARPKD?

The early signs of autosomal recessive PKD usually appear during the first few days of life or when the baby is in the womb. Enlarged kidney: An early sign of ARPKD is enlarged kidney size due to the thousands of cysts over the kidneys. 


Growth failure: Due to decreased kidney and lung function, babies with ARPKD are usually smaller than the average size. 


Low levels of amniotic fluid: Decreased kidney function often leads to low levels of amniotic fluid in the babies. 


What tests are done?

Following tests are done to know if your baby has ARPKD:


  • Ultrasonography of the kidney, ureter, and bladder

  • CT scan or MRI of the brain

  • 2D ECHO for the heart

  • Genetic test
